Stuttering - Wikipedia, the free encyclopedia. Stuttering () or stammering () (alalia syllabaris, alalia literalis or anarthria literalis) is a speech disorder in which the flow of speech is disrupted by involuntary repetitions and prolongations of sounds, syllables, words or phrases as well as involuntary silent pauses or blocks in which the person who stutters is unable to produce sounds. According to Watkins et al., stuttering is a disorder of . In the world, approximately four times as many men as women stutter, encompassing 7. This may include fears of having to enunciate specific vowels or consonants, fears of being caught stuttering in social situations, self- imposed isolation, anxiety, stress, shame, being a possible target of bullying (especially in children), having to use word substitution and rearrange words in a sentence to hide stuttering, or a feeling of . Stuttering is sometimes popularly seen as a symptom of anxiety, but there is actually no direct correlation in that direction (though as mentioned the inverse can be true, as social anxiety may actually develop in individuals as a result of their stuttering). Acute nervousness and stress do not cause stuttering, but they can trigger stuttering in people who have the speech disorder, and living with a stigmatized disability can result in anxiety and high allostatic stress load (i. Stutterers often find that their stuttering fluctuates and that they have . The times in which their stuttering fluctuates can be random. There are many treatments and speech therapy techniques available that may help decrease speech disfluency in some people who stutter to the point where an untrained ear cannot identify a problem; however, there is essentially no cure for the disorder at present. The severity of the person's stuttering would correspond to the amount of speech therapy needed to decrease disfluency. For severe stuttering, long- term therapy and hard work will be required to decrease disfluency. These differ from the normal dysfluencies found in all speakers in that stuttering dysfluencies may last longer, occur more frequently, and are produced with more effort and strain. Each of these three categories is composed of subgroups of stutters and dysfluencies. People who stutter commonly report dramatically decreased disfluency when talking in unison with another speaker, copying another's speech, whispering, singing, and acting or when talking to pets, young children, or themselves. It has been described in terms of the analogy to an iceberg, with the immediately visible and audible symptoms of stuttering above the waterline and a broader set of symptoms such as negative emotions hidden below the surface. Many perceive stutterers as less intelligent due to their disfluency; however, as a group, individuals who stutter tend to be of above average intelligence. Such negative feelings and attitudes may need to be a major focus of a treatment program. People who stutter may experience varying disfluency. In stuttering, it is seen that many individuals do not demonstrate disfluencies when it comes to tasks that allow for automatic processing without substantial planning. For example, singing . Tasks like this reduce semantic, syntactic, and prosodic planning, whereas spontaneous, . A to Z List of Services. Roles and Responsibilities of Speech-Language Pathologists in Early Intervention: Technical Report Ad Hoc Committee on the Role of the Speech-Language Pathologist in Early Intervention. Some researchers hypothesize that controlled- language activated circuitry consistently does not function properly in people who stutter, whereas people who do not stutter only sometimes display disfluent speech and abnormal circuitry. A variety of hypotheses and theories suggests multiple factors contributing to stuttering. It always arises from repetition of sounds or words. Young children like repetition and the more tense they are feeling, the more they like this outlet for their tension - an understandable and quite normal reaction. They are capable of repeating all types of behaviour. The more tension that is felt, the less one likes change. The more change, the greater can be the repetition. So, when a 3 year old finds he has a new baby brother or sister he may start repeating sounds. The repetitions can become conditioned and automatic and ensuing struggles against the repetitions result in prolongations and blocks in his speech. More boys stammer than girls, in the ratio of 3- 4 boys : 1 girl. This is because males have evolved as the hunters, fighters and as protectors and suitors. To succeed, they have required more adrenaline. Their Hypothalamic- Pituitary- Adrenal (HPA) Axis is more active. Whilst they are pumping out more cortisol than females under the same provocation, they can be tense or anxious and become repetitive. Researchers estimated that alterations in these three genes were present in 9% of people who stutter who have a family history of stuttering. These may include physical trauma at or around birth, learning disabilities, as well as cerebral palsy. In other people who stutter, there could be added impact due to stressful situations such as the birth of a sibling, moving, or a sudden growth in linguistic ability. Research is complicated somewhat by the possibility that such differences could be the consequences of stuttering rather than a cause, but recent research on older children confirms structural differences thereby giving strength to the argument that at least some of the differences are not a consequence of stuttering. Stuttering is less prevalent in deaf and hard- of- hearing individuals. In addition reduced activation in the left auditory cortex has been observed. In this approach, speech performance varies depending on the capacity that the individual has for producing fluent speech, and the demands placed upon the person by the speaking situation. Laura Bogart's work has appeared in various journals and she is a regular contributor to DAME magazine. She has completed a novel titled 'Don't You Know That I Love You?'. Roles and Responsibilities of Speech-Language Pathologists in Early Intervention: Guidelines Ad Hoc Committee on the Role of the Speech-Language Pathologist in Early Intervention. The Son-Rise Program Intensive: Advanced Training Program. While our Child Facilitators provide intensive input and. 4/13/2015 3 ISHA 2015 13 Capacity for fluent speech may be affected by a predisposition to the disorder, auditory processing or motor speech deficits, and cognitive or affective issues. Demands may be increased by internal factors such as lack of confidence or self esteem or inadequate language skills or external factors such as peer pressure, time pressure, stressful speaking situations, insistence on perfect speech, and the like. In stuttering, the severity of the disorder is seen as likely to increase when demands placed on the person's speech and language system exceed their capacity to deal with these pressures. Several neuroimaging studies have emerged to identify areas associated with stuttering. In general, during stuttering, cerebral activities change dramatically in comparison to silent rest or fluent speech between people who stutter and people who do not stutter. There is evidence that people who stutter activate motor programs before the articulatory or linguistic processing is initiated. Brain imaging studies have primarily been focused on adults. However, the neurological abnormalities found in adults does not determine whether childhood stuttering caused these abnormalities or whether the abnormalities cause stuttering. In single- word- recognition tasks, people who do not stutter showed cortical activation first in occipital areas, then in left inferior- frontal regions such as Broca. The people who stutter also first had cortical activation in the occipital areas but the left inferior- frontal regions were activated only after the motor and premotor cortices were activated. They show underactivity in the ventral premotor, Rolandic opercular and sensorimotor cortex bilaterally and Heschl. A large amount of white matter is found in the right hemisphere of the brain, including the region of the superior temporal gyrus. This was discovered using voxel- based morphometry (VBM). On the other hand, lesser amounts of white matter are found in the left inferior arcuate fasciculus connecting the temporal and frontal areas in stuttering adults. Men who stutter seem to have more right- sided motor connectivity. On the other hand, stuttering women have less connectivity with the right motor regions. By contrast, people who stutter yield more activity on the right hemisphere, suggesting that it might be interfering with left- hemisphere speech production. Another comparison of scans anterior forebrain regions are disproportionately active in stuttering subjects, while post- rolandic regions are relatively inactive. The corpus callosum, rostrum, and the anterior mid- body sections are larger in adults who stutter as compared to normally fluent adults. This difference may be due to unusual functions of brain organization in stuttering adults and may be a result of how the stuttering adults performed language- relevant tasks. Furthermore, previous research has found that adults who stutter show cerebral hemispheres that contain uncommon brain proportions and allocations of gray and white matter tissue. As a result, diagnosing stuttering requires the skills of a certified speech- language pathologist (SLP). Diagnosis of stuttering employs information both from direct observation of the individual and information about the individual. Information from both sources should span multiple, various settings and times. They may also observe parent- child interactions and observe the speech patterns of the child's parents. In particular, the therapist might test for factors including the types of disfluencies present (using a test such as the Dysfluency Type Index (DTI)), their frequency and duration (number of iterations, percentage of syllables stuttered (%SS)), and speaking rate (syllables per minute (SPM), words per minute (WPM)). They may also test for naturalness and fluency in speaking (naturalness rating scale (NAT), test of childhood stuttering (TOCS)) and physical concomitants during speech (Riley. One such test includes the stuttering prediction instrument for young children (SPI), which analyzes the child. Children and adults are monitored and evaluated for evidence of possible social, psychological or emotional signs of stress related to their disorder. ABA Schools and Centers . Speed limits may be variable and in some places speeds are unlimited. Speed limits are normally indicated on a traffic sign. Speed limits are commonly set by the legislative bodies of nations or provincial governments and enforced by national or regional police and / or judicial bodies. The highest posted speed limit in the world is 1. Poland. Best known are Germany's less congested Autobahns. It is often done with an intention to improve road traffic safety and reduce the number of road traffic casualties from traffic collisions. In their World report on road traffic injury prevention report, the World Health Organization (WHO) identify speed control as one of various interventions likely to contribute to a reduction in road casualties. Some cities have reduced limits to as little as 3. For some classes of vehicle, speed limiters may be mandated to enforce compliance. Impaired Driving (alcohol/drugs/fatigue)www. Use of any psychoactive (mind- altering) drug makes it highly unsafe to drive a car and is illegal—just like driving after drinking alcohol. Drugged driving puts at risk not only the driver but also passengers and others who share the road. Studies in several localities have found that approximately 4 to 1. THC. For instance, in a 2. Maryland shock trauma center, drugs other than alcohol were present in more than half of the cases. These included marijuana (2. A quarter of the cases involved both alcohol and other drugs.. One difference is the acceptable limit of blood alcohol content before a person is charged with a crime. Driving while either intoxicated or drunk is dangerous and drivers with high blood alcohol content or concentration (BAC) are at greatly increased risk of car accidents, highway injuries and vehicular deaths. Possible prevention measures examined here include establishing DWI courts, suspending or revoking driver licenses, impounding or confiscating vehicle plates, impounding or immobilizing vehicles, enforcing open container bans, increasing penalties such as fines or jail for drunk driving, and mandating alcohol education. Safety seat belts, air bags, designated drivers, and effective practical ways to stay sober are also discussed. THE PROBLEM. Every single injury and death caused by drunk driving is totally preventable. Although the proportion of crashes that are alcohol- related has dropped dramatically in recent decades, there are still far too many such preventable accidents. Unfortunately, in spite of great progress, alcohol- impaired driving remains a serious national problem that tragically effects many victims annually. Therefore, this page is dedicated to the memory of one randomly- selected victim of a drunk driver, young Holli Crockett. On the other hand, while only a few drivers have BACs higher than . High BAC drivers tend to be male, aged 2. DWI convictions and polydrug abuse. Drunk driving, like most other social problems, resists simple solutions. However, there are a number of actions, each of which can contribute toward a reduction of the problem. DWI courts, sometimes called DUI courts, sobriety courts, wellness courts or accountability courts have proven effective in reducing the crime of drunken driving (driving while intoxicated or while impaired). Such courts address the problem of hard- core repeat offenders by treating alcohol addiction or alcoholism. The recidivism or failure rate of DWI courts is very low. Automatic license revocation appears to be the single most effective measure to reduce drunk driving. Automatic license revocation along with a mandatory jail sentence appears to be even more effective than just automatic license revocation. Impounding or confiscating license plates. Mandating the installation of interlock devices that prevent intoxicated persons from starting a vehicle. Implementing social norms programs that correct the misperception that most people sometimes drive under the influence of alcohol. Passing mandatory alcohol and drug testing in fatal crashes would promote successful prosecution of drunk and drugged drivers. The National Highway Traffic Safety Administration has estimated that 1. For example. A study of drivers admitted to a Maryland trauma center found that 3. Think When You Drink sign. A study by the Addiction Research Foundation of vehicle crash victims who tested positive for either legal or illegal substances found that just 1. In a large study of almost 3,4. In the State of Maryland’s Adolescent Survey, 2. Intoxicated handwriting graphic. MADD Canada is to be commended for recognizing this serious but generally unrecognized problem and including the reduction of drugged driving as a major goal. Of course, fighting drugged driving must not detract us from working to reduce drunken driving. Passing and enforcing bans on open containers would probably reduce drunk driving by deterring drinking while driving. Surprisingly, some states have vehicular no open container laws. Imposing graded or multi- tiered penalties based on BAC at the time of arrest. This policy is virtually universal with regard to speeding. Restricting nighttime driving by young people. This appears to be effective in those states with such restrictions. Electronically monitoring repeat DWI offenders. Involving drivers in identifying and reporting possibly drunken drivers to law enforcement authorities by dialing 9. See Help Police Stop Drunken Drivers. Requiring every state to provide adequate information on alcohol and driving to prospective drivers and adequately testing them on the subject in their driver's exams. In too many states, the subject is given only brief mention and do not include any information or testing in the process of obtaining a driver's license. Some actually provide factually incorrect information. Jail or prison sentences for alcohol offenses, in spite of their great popularity, appear to be of little value in deterring high BAC drivers. Large fines appear have little deterrent effect, according to research. Increasing the cost of alcohol with increased taxation would have virtually no impact on reducing drunk driving. Both research and common sense suggest that heavy drinkers are not deterred by cost and most minors who drink don’t pay for or purchase their beverages. Improved roads and vehicles can contribute significantly to increased highway safety. Technological improvements include raised lane markers, which are easier to see and also emit a startling sound when a tire wanders over them. Similarly corrugations along the edges of roads emit a sound when driven over, thus alerting inattentive drivers to their inappropriate location. Wider roads, improved street and highway lighting, break- away sign posts, brake lights positioned at eye level, door crash bars, and many other improvements can save lives and be cost- effective. Remember, it is usually themselves and their passengers who are harmed by drunk drivers. The risk of collision for high BAC drivers is dramatically higher than for a non- drinking driver. Illicit drugs are involved in a large proportion of traffic fatalities. The dangers posed when fatigued are similar to those when intoxicated. A drunk or fatigued driver has slowed reactions and impaired judgement. And a driver who nods off at the wheel has no reactions and no judgement! Drivers who drift off cause about 7. Don't use a car phone, put on make- up, comb your hair, or eat while driving. Drivers using cellular phones are four times more likely to have an accident than other drivers. Steer clear of aggressive drivers. Aggressive drivers may be responsible for more deaths than drunk drivers. The contents of the typical bottle or can of beer, glass of wine, or liquor drink (mixed drink or straight liquor) each contain virtually identical amounts of pure alcohol. When it comes to alcohol, a drink is a drink is a drink and are all the same to a breathalyzer. For more, visit Standard Drinks. If you are not sure, experiment at home with your spouse or some other responsible individual. Explain what you are attempting to learn. Most people find that they can consume one drink per hour without any ill effects. Also, experiment with the Blood Alcohol Educator, which is very informative and useful. Food, especially high protein food such as meat, cheese and peanuts, will help slow the absorption of alcohol into your body. If you gulp a drink, you lose the pleasure of savoring its flavors and aromas. If someone tries to force a drink on you, ask for a non- alcohol beverage instead. If that doesn't work, . Having a non- alcoholic drink between alcoholic ones will help keep your blood alcohol content level down, as does spacing out your alcoholic drinks. A good general guideline for most people is to limit. If you stay active you tend to drink less and to be more aware of any effects alcohol may be having on you. Some drinks, such as zombies and other fruit drinks, can be deceiving as the alcohol content is not detectable. Therefore, it is difficult to space them properly. Ask your physician or pharmacist about any precautions or prohibitions and follow any advice received. Intoxicated behavior is potentially dangerous and never amusing. Take their keys, have them stay the night, have them ride home with someone else, call a cab, or do whatever else is necessary - but don't let them drive! This encourages conversation and social interaction rather than heavy drinking. This de- emphasizes the importance of alcohol and also sends the message that intoxication is not desirable. If you plan to ask a friend or relative to act as bartender, make sure that person is not a drink pusher who encourages excessive consumption. Determine the scope of the problem. The illustrations in this article show a repair where a slab reinforcing bar was too near the surface, and the concrete cracked. Other possible problems may include the following. Properly installed concrete should have joints that prevent this unsightly cracking. For a 4- inch thick slab, joints should be spaced 8 to 1. If jointing is not sufficient, or improperly placed, cracks can occur. Typically these cracks are not structural and only need to be repaired for aesthetic purposes. Debris like chunks of wood floating out of the concrete while it is still plastic (wet). Damage caused by heavy objects being dropped on the slab before it is cured. Surface scaling caused by improper finishing or freezing temperatures during curing. Multi purpose concrete repair material Fine sand allows very thin repairs Vinyl resin elimiates the need for bonding adhesive. Vinyl Concrete Patcher is made of a special blend of vinyl resin, fine sand and Portland cement. California residents. Scaling is when parts of the concrete surface flake away. Use a ruler to determine the length and width of the area to be repaired if the condition does not define itself. This may include bumps, depressions, or ridges. Small areas of shallow surface irregularities may be concealed with a floor leveling compound, rather than chipping out and replacing the actual concrete. For small jobs, a chipping hammer will be all you need, but larger repairs are much easier with an electric chipping hammer or even a jack hammer. Professionals also use grinding machines to prep the surface for repairs. This will allow the new material to have a solid surface to adhere to. A wire brush is a good choice for this step. A stiff bristled broom or paintbrush will also work. Depending on the age and condition of the concrete chemical cleaning may be required, it is best to consult a pro. Mix the dry ingredients first, using a proportion of 1 part Portland Cement (type I or II) to 2 1/2 parts clean masonry sand. Note that some bonding agents are applied directly to the area to be patched, like glue, so follow the instructions on the manufacturer's label. Do not flood the area, but make sure all surfaces are dampened, to assist in adhering the new cement mixture, as well as keeping the mixture from drying too quickly. Dry surfaces simply draw the moisture from the cement mixture too quickly, and will cause it to shrink, and ultimately, to crack. Place the mixture into, or on the patch location, and work it into the space forcefully with the trowel to force out any air bubbles. Allow it to dry and become stiff for an hour or so, depending on humidity and temperature. This will flatten and smooth the surface, and cause cement paste to rise to the surface. Very large, deep areas may be floated with a magnesium float to bring more cement paste up. This paste is the material that forms the surface of the finished slab. At this point, you may need to splash a little water on the surface to slow the drying, as well as making the finishing process somewhat easier. Use the edge of your trowel to shave, or scrape off any of your cement mixture which has spread to the adjacent, finished surface. Another good choice for patching materials are hydraulic cements. They usually set up in less than 3. Summer Programs - American University Washington College of Law. This school offers 3 summer courses. Summer Schools in Europe Top destination.
